Healthcare Inspection Questionable Cause of Death VA Puget Sound Health Care System Seattle, Washington [08-02620-22] 11/12/08. The VA Office of Inspector General (OIG), Office of Healthcare Inspections reviewed allegations regarding the death of a patient in the community living center (CLC)1 at the VA Puget Sound Health Care System’s (VAPSHCS) American Lake (AL) Division. The purpose of this inspection was to determine the validity of the following allegations:
• The patient’s death was caused by equipment failure and not aspiration pneumonia.
• Staff did not communicate to the complainant that the patient “choked” and had to be suctioned prior to his death.
• The physician did not accurately annotate the cause of death (COD) on the official death certificate. VAOIG

VA Puget Sound's answer to improving service shows about the same level of honesty.
I called on the 14th of November asking for a dental appointment.
I was told that all the appointments for December had already been given out, but that I could be put on the waiting list for any cancellations that might occur. I asked if I could get an appointment for January sometime and was told that if I called back after the first of December that I might be able to get a January appointment. I asked if I could make an appointment now for in January and was told that I would have to call back on December the 1st. I asked if I could get an appointment for February o March or whenever and was told “no, we don‘t do that”. I asked if I could get a fee for service authorization and was told “no, budget issues“.
I spoke to Donita Lopez, a patient advocate at American Lake, and was told the same thing as above and that this was Director Johnson’s policy and could not be changed. I reiterated that I just wanted an appointment for whenever the next available appointment was, be it this year or the next. She said it couldn’t be done and that she would not try, so much for patient advocacy!
If I was to call on the 1st of December for the 15th of January; the elapsed time for me to receive service would be 46 days. If I were to be set up with that same January 15th appointment today (14 Nov) the elapsed time for me to receive service would be 62 days.
If I was not quick enough to call on the 1st of December then I was told I would have to call back the 1st of January for an appointment in February. So if I got an appointment then for say Feruary 15th then the elapsed time reported would be 46 days wherein to my teeth the elapse time would 93 days.
Fraudulent, bogus, slimy, scum bag, are a few of the cleaner words for what I think of this crooked behavior exhibited by this change in policy from the old way the dental clinic was run.
The IG needs to do more!
