Having Hip Surgery at a VA Hospital Can Mean Avoiding a Nursing Home Stay

How health care is paid for appears to impact outcome 02/25/10. Whether an individual is treated for a hip fracture at a hospital reimbursed by Medicare or is treated at a Department of Veterans Affairs Medical Center makes a significant difference in what happens to the patient after release from the medical facility, according to a large study which may contribute to the ongoing debate on how to pay for health care. The seven–year study, which appears in the February 2010 issue of the Journal of the American Medical Directors Association, found that while 49 percent of those who had a Medicare reimbursed procedure went to nursing homes, only 35 percent of those treated at VA facilities were discharged to a nursing home. The complete explanation for this difference requires future study of institutional differences, patient transitions between care settings, and other issues as well as reimbursement policies. VAOIG Inspection of Wilkes-Barre VA Medical Center Wilkes-Barre, Pennsylvania

Healthcare Inspection Alleged Quality of Care Issues Wilkes-Barre VA Medical Center Wilkes-Barre, Pennsylvania 02/24/10. The purpose of the review was to determine the validity of the following allegations: (1) the third lead wire was not placed during the implantation of a replacement pacemaker, as planned, (2) a post-operative surgical site infection was misdiagnosed, causing a delay in treatment, (3) the patient advocate failed to respond to a request that the surgeon not be assigned to the patient during a subsequent readmission (4) clinicians prematurely discharged the patient to a community nursing home, and (5) the medical center provided substandard care because of unqualified nurses and physicians and insufficient nurse staffing. We did not substantiate the allegations. We concluded that the patient received appropriate care and was properly discharged to the nursing home setting. We also concluded that the patient advocate responded appropriately. We determined that the physicians and nurses practiced in a competent manner and that the nursing units were adequately staffed. However, we concluded that communication and documentation could be improved to ensure a clear understanding of the plan of care by the patient and family. VAOIG

Evaluation of Veterans' Disability Benefits Program

Veterans' Disability Benefits: Opportunities Remain for Improving Accountability for and Access to Benefits Delivery at Discharge Program 02/24/10. Although VA awards disability benefits more quickly under BDD than through its traditional disability claims process, gaps in program management and accountability remain. For example, VA does not separately measure the total time its personnel spend developing BDD claims. As a result, VA has limited information on potential problems and improvement opportunities regarding BDD claims. GAO continues to believe that VA should measure BDD development time; however, VA told GAO it has no plans to capture this information. GAO also found that VA implemented two initiatives to improve the BDD program—i.e., consolidating BDD processing in two offices and instituting paperless processing of BDD claims to increase efficiencies and improve security of information—but did not evaluate whether or the extent to which desired improvements resulted. Finally, GAO found that VA was not completely or consistently monitoring BDD operations at all locations. VA has since taken steps to review BDD operations at more sites and has revised its protocols to ensure more consistent reviews of BDD operations. GAO

VA Cans 12 Health IT Projects, Reinstitutes 32 Others

VA kills 12 health IT projects, puts 32 back on track 02/24/10. Several health IT projects at the Department of Veterans Affairs have been canned following evaluations performed under a stringent project management program while still others have earned a new lease on life as a result of the same program. On a conference call today, Roger Baker, the VA's assistant secretary for information and technology, said that evaluations under the Program Management Accountability System (PMAS), which was inaugurated last summer, led to the termination of 12 projects and to the restarting of 32 of them. Adult Stem Cells for Spinal Cord Injuries

UTHealth research shows modified adult stem cells may be helpful in spinal cord injury 02/24/10. Researchers at UTHealth have demonstrated in rats that transplanting genetically modified adult stem cells into an injured spinal cord can help restore the electrical pathways associated with movement. The results are published in the Feb. 24 issue of the Journal of Neuroscience. Advantages of Electronic Patient Records

Medical Paper Trail Takes Electronic Turn‎ 02/23/10. President Obama’s economic stimulus included $19.2 billion for health information technology, and a number of hospital systems around the country are taking advantage of this financial incentive and investing their own money to foster the creation and use of electronic records that are expected to improve the quality, efficiency and safety of medical care.

Dr. Litvin, now an internist at the Medical University of South Carolina who teaches doctors how to improve care using electronic records, pointed out that the Veterans Health Administration has used its computerized patient record system for the past decade, and its doctors report that it has greatly improved the care of some eight million veterans. Prescriptions, laboratory tests, studies, consults, reports and progress notes from all visits by patients to any Veterans Affairs hospital are stored in each patient’s electronic record. Female Veterans Continue to Battle Even After Returning Home

Lethal Legacy of Agent Orange

Perry has never been to Vietnam, what happened to her husband, who served there during his military service, has affected her life forever. Since he died in 2005 she has “waded through grief, hate, anger, pain, rage, and lots and lots of heartache.”

The heartache started long before Reuben “Bud” C. Perry III died from a myocardial infarction attributed to his diabetes, peripheral neuropathy, and Hep C, all Agent Orange (AO) related illnesses. It began with the birth of their oldest daughter, Danielle, who has been sick all her life.
